As a Triage Nurse Practitioner or Physician Assistant , you'll examine patients, diagnose injuries and illnesses, and provide treatment to H-E-B Partners and their families, ensuring the highest quality patient-centered care. You'll work with an integrated team of healthcare providers, focusing on complex chronic care patients with the goal of improving patient health outcomes. You’ll also support clinic operations by providing patient care for other routine and acute care visits.

We are looking for:
Nurse Practitioner:
- a master's degree in advanced practice nursing from an accredited program; 3+ years of experience
- current, valid State of Texas RN & APRN licenses; current, unrestricted DEA
- advanced interpersonal and counseling skills; ability to work as part of an integrated healthcare team to provide an exceptional patient experience
or
Physician Assistant:
- a graduate degree from an accredited PA Program; 3+ years of experience
- Board Certification by the NCCPA; current, unrestricted license to practice in Texas
- current, unrestricted DEA
- advanced interpersonal and counseling skills; ability to work as part of an integrated healthcare team to provide an exceptional patient experience
What is the work?
Patient Care:
- Provide triage and primary care services to patients (ages 12 & up) through in-person and tele-health visits
- Ability to triage patients based on severity of condition
- Conduct services for patients with chronic medical conditions to improve quality patient care and reduce hospital readmissions and unnecessary emergency room visits
- Interviews patients; takes medical histories; assesses patient health; performs physical examinations; administers injections/ immunizations; performs suturing, splinting, casting; manages wounds and infections
- Instructs / counsels patients; explains treatments and medications; counsels / promotes emotional and physical health and wellness
- Educate the patient on properly contacting the physician's office for urgent care matters / or 911 when appropriate or unsure
- Work closely with the triage team to manage the day-to-day calls involving: symptom control, and medication management, and provide patient and family education.
- Obtains / studies medical histories; documents patient and clinic records
- Administers / orders diagnostic tests; interprets results; makes diagnoses; prescribes medications and other treatments
- Identifies short- / long-range patient care issues; recommends courses of action; implements physician directives
- Provides / ensures continuity of care; develops / implements patient care management plans
- Complies / ensures compliance with federal, state, and local legal and professional requirements, and infection control protocols and policies
- Develops health care team staff; provides training and educational / growth opportunities
- Maintains professional / technical knowledge; reviews professional journals; establishes personal network; participates in educational workshops and professional associations
- Collaborates with clinic staff to recommend / improve patient care practices, and clinic guidelines and workflow
- Ensures adherence to HIPAA regulations
What is your background?
Nurse Practitioner
- Master's degree in advanced practice nursing from an accredited program
- 3+ years of experience in primary care as a provider
- Current valid RN License
- Current, unrestricted State of Texas APRN license
- Current, unrestricted DEA
- AHA BLS Healthcare Provider or AHA BLS Instructor Provider card
or
Physician Assistant
- Graduate degree from an accredited PA Program
- 3+ years of experience in primary care as a provider
- Board Certification by the NCCPA
- Current, unrestricted license to practice in Texas
- Current, unrestricted DEA
- Current BLS certification by an approved American Heart Association organization
